Description
Perinatal Health Partnership Program is a home visiting program for high risk pregnant and postpartum patients and their infants. The goal of perinatal home visiting services are to improve maternal and infant outcomes through clinical assessment and monitoring, education, linkage to needed services and resources, and timely identification and referral for warning signs and symptoms. This program will provide home visiting services for up to one-year post-partum.
This is a full time, full benefit eligible position, located in Lowndes County, Georgia. This position is Grant Funded and will be considered for renewal on a year to year basis.

Job Responsibilities
Under supervision, provides professional nursing care in accordance with nurse protocols, policies and/or procedures.
Additional Duties:
Responsible for home visits according to program guidelines which include clinical assessments with special emphasis on fetal heart tones. This position will also be responsible for patient education, counseling, provider and community outreach.
Minimum Qualifications
Associate's degree in nursing from an accredited college or university AND completion of a PHN course within one year of hire AND current Georgia Registered Professional Nurse license AND CPR certification. Note: No experience required.
Preferred Qualifications: Bachelor's Degree in Nursing
