Primary Responsibilities <ul><li>Maintain a case load of 12 families, providing direct face to face contact with youth and families in accordance with the Family Voices Network and Erie County Preventive Services.</li><li>Coordinate services for families enrolled in the Erie County Wraparound Program which includes oversee the aspects of Care Coordination for families enrolled in Wraparound Services</li><li>Facilitate all Child and Family Team Meetings (CFT) and ensuring all relevant participants are included</li><li>Communicate with all necessary providers and supports related to the planning and care for youth/families</li><li>Fulfill other duties as assigned</li></ul> Required Qualifications <ul><li>Bachelor’s Degree in Human Services related field with 4 years’ experience providing supportive services to children and their families</li><li>Current NYS Driver’s License and personal vehicle</li></ul><ul><li>Ability to build rapport with youth based on healthy boundaries, effective communication and mutual respect</li><li>Ability to exercise discretion and judgement in decision making</li><li>Ability to demonstrate sensitivity, compassion, patience, and active listening</li><li>Ability to effectively work with individuals from a diverse array of cultural backgrounds, beliefs, customs, and lifestyles</li></ul> Preferred Qualifications <ul><li>Master’s degree in Human Services related field with 2 years’ experience providing supportive services to children and their families</li></ul> We want to emphasize that the preferred qualifications are not required and that we are committed to helping our future colleagues develop these preferred skills.
